Birth Registration
The birth registration service operates strictly by appointment - see office opening hours at bottom of this page.
We would request that parents scan and email or forward a picture of the GRO4 form (see below link or editable version further below) if they can or post to the relevant registration office to enable us to input the information to our system in advance and make contact to arrange an appointment. https://www.nidirect.gov.uk/publications/birth-registration-form
Births must be registered by law within 42 days from the date of the birth.
This can be effected by attending any District Registrar’s Office in Northern Ireland.
Once the registration is complete, the Registrar will issue the following forms:
One free short birth certificate
HS123 Infant Registration form to register with the Family Doctor and obtain a medical card.
Further certified copies (short or full) may be purchased from the Registrar at the time of registration for the reduced fee of £8.
Office opening hours are:
Ballymoney, Coleraine and Limavady: Monday – Friday: 9.30 am – 12.30 pm and 2.00 pm – 4.30 pm
Ballycastle: Monday & Friday 1.30 pm – 4.30pm. Tuesday, Wednesday and Thursday 9.30am – 12.30pm
An appointments system operates.
